Each applicant for a license shall complete and sign an application on forms provided
by the City, except as otherwise provided in this chapter. Such forms shall require
such information as the Director shall deem necessary to determine the proper license
tax to be paid by the applicant, including the name and address of the individual
if a single proprietor, the names and addresses of all owners if a firm, or the names
and addresses of all principal officers if a corporation, the name of the business,
the address at which the business is to be conducted, the type of business, and the
activities to be carried on for which the license is being sought. Upon determining
the license tax due, the Director shall indicate the amount on the application form,
and upon payment of the tax, shall issue to the applicant a receipt for taxes paid.
